This pushes a fine line on medical ethics but political class is not a protected class against which you can discriminate, however, the doctor isn't quizzing people on their political affiliation nor has he turned people away, just making his views known and leaving people free to come or go.
BUT also: Doctors have free speech too you know!
Me personally, I think if you loved obamacare you should be allowed to have it and pay the taxes on it, but if you don't want it, you're exempt.
